    Otsego Local Schools Administrative Note:
  
In order to start/complete enrollment process, the following will need to be completed.
 
1. Please complete all forms in Final Forms.  Every form must be green in color on the parent side.
 
2. Please submit Proof of Residency.  This will need to be a utility bill or mortgage/rental agreement with your name (the guardian/residential parent) on it.
--If you do not have these documents in your name, then the homeowner will need to get a NOTARIZED statement stating you and the children live there.
 
3. Court documents stating you are the residential parent of the children if applicable.
 
4. Please have ready a copy of the child's Birth Certificate and Immunizations.
 
Once the forms are completed and I receive the documents, I will then request records from their previous school. 
 
***Until then, they are considered students of their previous school and won't be withdrawn from that school until I request an official record request.
